Passport Service at the Moline Public Library
 

The Moline Public Library is an authorized United States Passport Acceptance Facility and accepts passport applications on behalf of the U. S. Department of State.

We only offer processing for first-time applicants for a passport book and/or passport card. Passport renewals are not accepted. 

We are not a Regional Passport Agency and do not provide physical passports on site.

 

PASSPORT APPLICANTS

All applicants and relevant parties must be present during the application process.

Both parents and children must be present for all minor applications (under age 16). If one parent cannot be present, you must submit a Parent Consent Form (DS-3053), completed with a notarized signature from the non-applying parent/guardian.

 

HOURS OF SERVICE

Passport service is offered by appointment only. The last appointment of the day is 60 minutes prior to closing. Passport Agents reserve the right to stop accepting applications prior to that time, if it means that the processing service will continue past closing.  Please allow adequate time for each application.

REQUIRED DOCUMENTS

Each applicant must provide the following:

  • Current version of passport application form DS-11 (completed and unsigned)
    Complete form online (travel.state.gov) and print OR print a blank form and fill out using black ink only.  Application forms are also available at the library.
  • Fill out application completely, including social security number. Applications without SSN cannot be processed.
  • Do not sign your application until asked to do so by a passport agent, who must administer an oath and witness your signature.
  • Evidence of citizenship
    Original or state certified copy of birth certificate (not a hospital certificate or photocopy), a previous passport, naturalization certificate, or a consular report of birth abroad.
  • Proof of identity
    Current driver’s license, state ID card, military ID card, or government employee ID.
  • Photocopy of your identification document
    Copy must have both front and back of ID.
  • One recent 2”x2”color, full-face view photo
    Must have been taken within the last six months, with a white or off-white background, with normal street attire. No hats, headgear, or uniforms are permitted.

 

FEES

Two separate payments are required:

Check or money order, payable to Department of State, for each application (see fees below)

Check or money order, payable to Moline Public Library, for each application processed. 

Separate checks or money orders are required for multiple applications. The library does not accept cash, credit cards, or debit cards for processing fees.  A $30 fee will be charged for returned checks.

PASSPORT STATUS

Routine applications are processed within 4 to 6 weeks.  During peak times (March through September), processing times can increase to 8 weeks or more.

To check the status of your application call 1-800-487-2778 or visit travel.state.gov.  The library is unable to check passport status.
